1. DISTRICT DRIVER'S FORM

Print and complete the form.

2. ICBC PERSONAL DRIVING RECORD

Request your personal driving record from ICBC. Print.

3. PHOTOCOPY DRIVER'S LISENCE AND INSURANCE POLICY.

A minimum of 2million liability is required. Please photocopy your license and insurance liability page.
